If you live in our practice area and would like to register with us, please complete one of our registration forms, available from our reception counter. You will need to show up-to-date proof of residence (e.g. a recent utility bill). Or you can do this online - see the next tab.
When you register, you will also be asked to fill out a medical questionnaire. This is because it can take a considerable amount of time for us to receive your medical records. Or you can do this online - see the next tab.
Newly registered patients who are on medication should make an appointment to see a GP well before your medication will need renewing.
Medical treatment is available from the date of registration.
You will be able to say which practitioner you would prefer to see. However, you will be registering with the practice rather than an individual GP.
Our practice list is open for registrations. Our main practice boundary covers all of Penwortham, and extends to Howick and Kingsfold. We can also register patients who commute to Preston, but live outside of the area, as long as the patient is generally fit and well and unlikely to need home visits.
We accept registrations for all UCLan students.
