If you live in our practice area – which you can see on the map below – and would like to register with us, We are using a new online service called Register with PSM Medical Group (St Mary’s Health Centre) that makes it easy to register with this GP surgery.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one and you can contact our reception team on 01772 970079.
Newly registered patients who are on medication should make an appointment to see a GP before your medication will needs renewing, though medical treatment is available from the date of registration.
You will be able to say which practitioner you would prefer to see. However, you will be registering with the practice rather than an individual GP.
If you are outside of our practice area you may still be able to register as an ‘Out of Area’ patient by completing the online form, but we may need to contact you before we confirm your registration. Being an out of area patient means we will not be able to provide a home visit should you need one and we will also need you to complete an additional form.
